Dr. Mark Richards, a plastic surgeon in Washington, DC, recently returned from lecturing to the 4th M.E.G.A. Hand International Symposium and the 7th European Masters in Aesthetic & Anti-Aging Medicine Conference in Paris, France. As one of the pioneers of minimal incision arm lift or minimally-invasive brachioplasty (MIB), as well as optimal treatment of menopause with testosterone alone, Dr. Richards’ speaking topics covered performance of the MIB procedure as well as the need for testosterone for anti-aging and the anti-inflammation needs as men and women age.

At the recently held 4th M.E.G.A. Hand International Symposium in Paris, Dr. Mark Richards delivered lectures covering the latest techniques based on his experience and qualifications in plastic surgery in Washington, DC. Dr. Richards focuses on a wide range of procedures in his practice, ranging from facial rejuvenation and body sculpting to breast augmentation in Washington, DC. His topics at the Paris MEGA Hand conference focused on his years of experience and advancements in minimally-invasive techniques for brachioplasty (arm lifts). The minimally-invasive arm lift, performed in conjunction with liposuction, treats patients who have loose, excess skin hanging on the upper part of the arm. Dr. Richards’ innovations in brachioplasty offer smaller incisions, which he says leads to less scarring and more effective results.

Dr. Richards then delivered a presentation on the use of testosterone time-released pellets for the optimal treatment of female menopause. He says the anti-aging medical practitioners attending the lecture were fascinated by the simple, albeit revolutionary use of testosterone in women’s health.

As a frequent speaker at international medical conferences, Dr. Richards says he hopes the lectures inspire future “outside the box” thinking in plastic surgery procedures and in the advancement of new techniques and technology in the field. He also adds that as innovation in the plastic surgery industry continues, the quality of patient care and results will improve as well. “Without creative applications of scientifically proven evidence based medical therapies, minimal advancements in health delivery will occur. Conferences such as these provide the necessary fertile ground to allow the exchange and development of new ideas.”

At his Washington DC plastic surgery practice, Dr. Mark Richards discusses the latest statistics from the American Society for Aesthetic Plastic Surgery (ASAPS), which show more than 50% of Americans now approve of cosmetic enhancement. While he believes the increasing approval may be due to the rise in popularity of non-surgical enhancement, he stresses the results many of his patients desire are often only attainable through traditional procedures, such as the cheek lift, neck lift, or facelift in Washington DC.

Dr. Mark E. Richards is a board-certified plastic surgeon in Washington DC. As highlighted in the most recent report from the American Society for Aesthetic Plastic Surgery (ASAPS), more than half of all Americans now approve of cosmetic enhancement; a fact that Dr. Richards says is likely due to the surge in popularity and success of non-surgical treatments, such as dermal fillers or BOTOX Cosmetic®. However, he stresses the need to discuss a patient’s aesthetic enhancement goals and expectations in detail, as non-surgical procedures often do not achieve the transformation attainable with more classic procedures such as eyelid surgery or a facelift.

While non-surgical procedures accounted for 83% of the more than 9.3 million cosmetic procedures performed in 2010, Dr. Richards says the statistics from ASAPS—data that is also reflected in his plastic surgery practice— show traditional procedures are still very prevalent, and depend wholly on patient expectation and desires. Despite the declining number of facelifts performed, he highlights the increase in the number of combined eyelid and cheek lift surgeries performed, making eyelids the third most performed procedure in 2010. “Often times a patient new to fillers will request fillers believing it will give them an almost magical anti-aging result, when in reality a much more beautiful, long lasting, and natural improvement can be made with cheek lift and eyelid surgery. With the natural cheek curves restored, often just a neck lift replaces the full facelift for later surgical rejuvenation with much less down time.”

Ultimately, Dr. Richards says he believes the increase in acceptance of cosmetic procedures will continue to rise as cosmetic enhancement becomes more affordable and requires less downtime. He remains committed to working with each patient to ensure they achieve the results they desire, while providing the safest and most effective treatment available.

I wanted to start off by asking you Dr. Richards what Rhinoplasty is. Well Rhinoplasty is the changing of somebody’s nose. It’s as simple as that. And in what shape or form are we talking about when we say changing one’s nose? Well sometimes the nose is too small proportionally for the face, sometimes it’s too large. Sometimes there is just one area that may be aesthetically unattractive. There’s different approaches that can be used for Rhinoplasty. You can do a closed Rhinoplasty meaning all the incisions are inside the nose or you can do an open Rhinoplasty where most of the incisions are inside the nose but there’s one incision on the outside right about here, and you use that incision to lift the skin up off the nose to work on the structures directly. That’s called an open Rhinoplasty. Closed Rhinoplasty has been around for centuries. Open Rhinoplasty was really popularized perhaps about 10-15 years ago.

Can you please begin by telling us what the cheek lift is for those of us who may not know? Well quite simply the cheek lift is the most powerful method of rejuvenating the face with the shortest recovery, the shortest amount of surgery. And what exactly does a cheek lift do to one’s face? Cheek lift concentrates on the mid face area, the lower eyelids and the cheeks. As we age, it could be in our 30s or 40s, our cheeks tend to drift downwards. They drift downwards and into the mouth. It causes down turning of the mouth, an unhappy look, and lengthening of your lower lids. They get longer and longer. And how long has this been available? Well technically cheek lifts have been available as long as we’ve had cheeks but as far as the real plastic surgery procedure I would say we started focusing on it in the late 80s, early 90s. But it wasn’t until maybe the mid or late 90s that the procedure that I currently use was described and really has become popularized.

Who is a typical candidate for the cheek lift procedure? I’ll tell you, this is just amazing to me. The longer I do them the wider the audience I think that it’s applicable to. I’ve done people as young as in their early 30s, and I’ve applied it to people in their 70s. Ideally I like to do this procedure before the entire face needs to be rejuvenated. I certainly can do it and often do it as part of face lift and brow lift and eyelids and neck lift, etc., but it’s really a wonderful thing to do to somebody in their 30s or 40s or even early 50s before other areas of the face start to age.

The first patient that we’re looking at is a woman in her 20s who came to me and as you can see from the picture on the left she has a curvature to her nose, she has excessive width of her dorsum, the boning part of the nose, and she has some fullness of the nasal tip. On the right hand picture you can see that the nose is slightly narrowed. The nasal tip is natural but more defined. On the ¾ view you can see how before surgery the nasal tip not only didn’t have the cartilage definition but actually stuck out a bit. The top of the nose is flattened and wide. You can see the light reflections over the bones that were spread apart at the top, that’s just the way she was. And the after shot on the right, you can see the nasal dorsum is a smooth curve with an even light reflection coming right down the center of the nose where it should be. The nasal tip is slightly more rounded but more delicate at the same time. And the nasal tip is elevated ever so slightly. And finally on her side view you can see quite a change. Remember earlier I was discussing how you can have an indent up by the bridge of your nose that makes a hump look larger. I actually took down her nasal bones just a very tiny bit even though on the side view here it looks like she has a hump. What I did though is I filled in the indent with some cartilage, and you can see how it makes the nose look smaller even though I in fact added something to it.[See post to watch Flash video]

The first patient that we’re showing is a woman in her early 50s. We have a front view where you can see the length of the lower eyelids on the left and the shortening of the lower eyelids after the cheek lift on the right. On the ¾ view you can see how the cheek is flattened, the right cheek is flattened. On the right hand side you can see after the cheek lift the natural curve that’s restored there. And finally on the side view you can appreciate the flatness of the cheek on the left-hand side. When you look at the right-hand picture you can see how the cheek is swept upwards and has a light reflection on it. It’s a reflection of beauty and youth. Second patient is approximately 40. Picture on the left shows a tired, somewhat perhaps unhappy looking person even though she is not unhappy at all. On the right side she looks more like she’s in her 30s, happy, the eyelids are shortened, they’re smoother. On the ¾ view you can see on the picture on the left how the cheek is flattened and on the right how there’s some prominence and some light reflection and again the eyelids have been shortened considerably. And finally on the side view you can see that some neck work was done as well however the enhancement of the cheek exposes more of the lip and again raises the light reflection and gives her some prominence of the cheek that is symbolic of youth and happiness. The third patient is in her 40s but has tremendous issues going on with the lower eyelids and the cheek area. You can see that on front view on the left, and on the right hand picture the cheeks are up, the eyelids are much shortened, she has a happy expression, really reflecting how she feels inside. On the ¾ shot again you can see the pouching by the mouth, the downturn of the lips in the corner. On the left side and on the right side the mouth when she’s at rest is at a neutral expression, it’s not unhappy, and you can see the prominence of the cheeks. Again with her on the side view you can see the prominence of the lateral side of the cheek, the outside part of the cheek, being brought up.

Is there any discomfort involved with this procedure Dr. Richards? Really there’s not discomfort. There’s some changes in sensation of the lips and cheeks for up to a couple weeks afterwards because as you move the cheeks up the nerves get pulled a bit but as far as pain I don’t think I’ve had more than 1 or 2 patients in 8 years say that it was painful. So very minimal discomfort at best. Minimal discomfort. Alright.

And I believe you have some before and after pictures that you’re gonna show us. can you please describe them? Sure. In this first set of slides you see a patient holding her right arm out. The upper slide shows a before picture, the bottom slide shows her result after a year. On the second set of slides the upper pictures shows the back of a woman’s arm, that’s her left arm, and the bottom again is a year out showing the post operative results. On these third set of slides I took a close-up picture of the scar in the armpit two years out from surgery. Now the patients that we’ve seen so far have had a mild to moderate deformity of their upper arm that they complained about. This final set of photographs shows patients that have a more severe deformity with their upper arms. And you can see on the pictures on the right the result 1-4 months after minimal incision brachioplasty.

Finally your treat, your pictures that haven’t been seen before, the left is a front view of Miss Tripp approximately 6 months after she had her face lift surgery done in Los Angeles. She was very unhappy with it as you heard. I think she probably had some improvement from what she originally looked like but there were things about her face that really bothered her greatly. She didn’t like the chin implant that was given to her there that made her almost look like her nemesis Jay Leno. She has an elongated face and so she needed more width applied to the cheeks. That can only really be accomplished through cheek lift or a cheek implant. The brows were still down giving her a stern expression. So she had many of these things done, too much to go into today, and you can see on the right her face is much more oval, she’s softer, she has a much prettier look. In fact she looks pretty. You can see a ¾ shot which is really the most telling shot. It’s the view that we look at people the most in conversation. We’re rarely face to face and you’re rarely talking to somebody on their side. You’re almost always talking to somebody 3/4. It’s the least threatening, it’s the most common in conversation. And here is her ¾ shot. On the left you can see the cheeks are flat, she looks tired, she looks a bit sad, the chin is pointy and long. And on the right everything is much smoother. It’s more curved, more oval. And she has an attractive appealing look to her.
